A mobile health concept for young people living with chronic conditions. It brings health records, daily routines, peer support, professional consultation, family involvement, and emergency help into one coherent service.

Chronic disease management crosses physical, emotional, and social needs, yet care is frequently fragmented across tools and institutions. The experience needed to support daily independence without removing family and clinical connections.

The case demonstrates end-to-end UX thinking from research to service delivery. A next version would prioritize the minimum viable flow and validate privacy expectations with patients, family members, and clinicians.
